Complete the online nomination form and in addition please submit a citation outlining the key reasons why you are nominating the candidate – this should be no more than 500 words and should cover all the points in the category’s criteria and answer the questions related to that category.

You can use the text field below to provide your citation or upload it as part of your supporting information. The nomination will not be sent for judging if this citation is not provided.

You can include additional supporting information and examples of outstanding contributions to our industry, eg success stories in training; participation as either a competitor or a judge in competitions; accolades received; recognition from food guides and media; involvement in charitable or industry organisations; or samples of thank you letters from customers where the nominee is highly praised.

We can also accept video nominations in place of your written nomination.  This should answer the questions related to the category and can be either the nominee themselves in the video or you as the proposer.  Videos should be a maximum of 5 minutes in length.  Any additional supporting information can also be uploaded below.

Videos can also be submitted as supporting information using the same method.
